Under the direct supervision of the RN Case Manager, provides continuous care services, in-home respite care services, and personal care for the patient and assists with activities of daily living. Supports the Provider Relations activities of the organization.

Minimum Qualifications

* Licensed Practical Nurse/Licensed Vocational Nurse with current state license.

* 2 year’s of nursing experience with at least 1 year of home care experience preferred.

* CPR certified.

* Must have 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage.

* Must have valid driver’s license.

* Must be capable of maintaining regular attendance


Preferred Qualifications
* Must be able to maintain confidentiality regarding patient and company proprietary information.

* Must have the ability to relate professionally and positively and work cooperatively with patients families, and other employees at all levels.
